San Antonio Region Readmissions Learning Collaborative<\/a><\/em><\/p>\n

The first in the\u00a0Wrestling Readmissions<\/em><\/strong>\u00a0series will be held on June 10, 2015, as Gulshan Sharma, MD, MPH, FCCP offers insights into key interventions known to reduce readmission rates and shares early lessons from readmission programs.<\/p>\n

Dr. Sharma\u2019s presentation will focus on these key points:<\/p>\n

    \n
  1. Familiarization with key interventions that have shown to reduce readmission rates.<\/li>\n
  2. Understanding elements of project BOOST and its implementation using Health IT.<\/li>\n
  3. Examining early lessons learned on readmission project under DSRIP 1115 Waiver in Region 2 of the state of Texas.<\/li>\n<\/ol>\n

    \nSealy and Smith Distinguished Chair in Internal Medicine, Professor and Director of Division of Pulmonary, Critical Care and Sleep Medicine,\u00a0 Program Director for Pulmonary Critical Care Medicine Fellowship program, and Associate Chief Medical Officer for UTMB Health System<\/p>\n

    Dr. Sharma is a senior fellow in Sealy Center on Aging. His research interest includes health services research using large national databases and preparation of Health System. His current work is funded by UT system Health IT and system engineering grant. He is Co-investigation on R24 Patient centered Outcomes Research in elderly and recently funded NIA-PCORI grant on falls prevention in older adults.<\/p>\n

    \"\"Carol A. Huber, MBA<\/strong>
    \nDirector, Regional Healthcare Partnership<\/p>\n

    Carol Huber is the Director of Regional Healthcare Partnership with University Health System in San Antonio, Texas. Under the Texas Medicaid 1115 Waiver, she leads the anchor responsibilities for the 20 counties in Region 6 working to improve health and transform care. Carol has worked for the University Health System since 1999, implementing health promotion programs, providing process improvement consulting, and directing data analysis and strategic planning initiatives for the system-owned managed care plan. Carol earned a bachelor of science in community health from Texas A&M University and completed a master\u2019s degree in business administration from the University of Texas at San Antonio. She will begin work on a doctorate in public health through UT Health \u2013 Houston this fall.<\/p>\n
