Overview<\/h2>\n

The Institute for Integration of Medicine & Science (IIMS) at the University of Texas Health Science Center at San Antonio (UTHSCSA), in partnership with the University of Texas at San Antonio\u2019s (UTSA) Research Centers in Minority Institutions and Department of Biomedical Engineering, is pleased to host the 16th Annual Frontiers of Translational Science Research Day<\/strong> on April 27, 2026 <\/strong>at UT San Antonio – 7703 Floyd Curl Drive San Antonio Texas 78229<\/em>.<\/p>\n

This annual event brings together researchers, trainees, and scholars to share innovative work, foster collaboration, and celebrate excellence in translational science. We anticipate a day highlighting innovative research, meaningful collaboration, and scholarly exchange across our research community.<\/p>\n

Comprehensive Care for Patients at <\/span><\/strong>Increased Risk of Hospitalization<\/strong>
\n<\/span><\/em>Presented by our guest speaker
\nDavid Meltzer, MD, PhD<\/span><\/h6>\n

This talk will describe the rationale for and initial results from two clinical trials studying the Comprehensive Care Physician (CCP) Model, in which patients at increased risk of hospitalization can receive care from the same physician in clinic and in the hospital. In one version of the model, patients also receive a range of interventions to address unmet social needs. Evidence suggests the CCP model can significantly improve patient outcomes while reducing resource utilization. The talk will also describe progress disseminating the model.<\/p>\n
