Comprehensive Care for Patients at Increased Risk of Hospitalization
Presented by our guest speaker
David Meltzer, MD, PhD
This talk will describe the rationale for and initial results from two clinical trials studying the Comprehensive Care Physician (CCP) Model, in which patients at increased risk of hospitalization can receive care from the same physician in clinic and in the hospital. In one version of the model, patients also receive a range of interventions to address unmet social needs. Evidence suggests the CCP model can significantly improve patient outcomes while reducing resource utilization. The talk will also describe progress disseminating the model.
